Nashville: The Tennessee Historical Society, 1987. Hardcover. Small 4to. Green cloth, pictorial dust jacket. xiv, 327pp. Illustrations, maps. Near fine/good plus. Jacket edgeworn and a bit rubbed, with couple of archivally closed (on verso) edge tears. Tight, nice first edition, boldly signed by Durham in black ballpoint (along with "Best Wishes!") on the front flyleaf. Noted pediatric medicine professor and Civil War medicine scholar/collector Ralph C. Gordon's copy, with a small number of inked margin marks and underlinings by him.
