Some disease entities appear in several chapters and this has been done in order to ensure that each chapter is relatively self-contained and the reader does not have to browse through other chapters in order to cover the topic. In addition, the chapters are cross-referenced to guide the reader to another chapter where the same topic is also covered, usually in more detail.
