London: Morland Press, Ltd. for "Form", 1916. First edition. Small quarto, title-page, titles, initials in red, illustration by Austin Osmond Spare, original printed wrappers. Copy 68 of 122 copies printed on Italian hand-made paper out of a total claimed edition of 200 copies, although more were printed. Wade 114. Contains "The Dawn," "On Woman," "The Fisherwoman," "The Hawk," "Memory," "The Thorn Tree," "The Phoenix," and "There Is a Queen in China." Wrappers lightly rubbed and marked; bookplates of James Jerome Hill II and J.O. Edwards, stamp from the Fondation Camargo.
