Dark blue cloth with gilt lettering on the spine. 217 very clean unmarked and uncresed informative and educational pages! Wonderfully well preserved! Extremely rare and hard-to-find! Dj is nicely preserved in a brand new protective mylar plastic cover! Very very scarce title! "In 1958, under the auspices of the Health Information Foundation, and in collaboration with the National Opinion Research Center, a second large-scale survey was undertaken to gather information about family medical expenditures. The survey was national in scope with the sampling done on an area-probability basis. Those interviesed were asked about their private expenditures for health services during the previous year.....".
