Single long leaf (6.2x19.5 inches)of pink paperstock printed dark blue and folded five times to make 12 printed panels (6.2x3.3) filled with conditions of membership, list of late claims paid (name, city, nature of disability, amount) and excerpted letters of gratitude (name, city, date).
