Of the 185 copies produced and distributed by James F. Carr, 152 copies also contained a separate map volume with a large fold-out map with several pages of map keys with both volumes housed in a slipcase. James Carr had the maps made up in batches, as he was using his own money for the project, and delivered them to the bindery when they became available. The last set of maps (33 of them) never made it to the bindery, as financial and then health complications ended the project. The bindery was left with the last 33 copies without having been paid, but went ahead and bound them without the map volume. The spine on the last 33 copies have a green label to show the difference.
